Informal Stage
Step 1: Strike
Strikes are a form of informal warnings given to the staff for various things. Up to 3 strikes in 12 month period.

Step 2: Investigation Referal
This referal is for when you beleive that the staff member should have a disciplinary investigation carried out. This could be for conduct or getting 3 strikes.

Investigation Stage
Step 1: Investigation Notification
This will notify the member of staff that they are under investigation. This will give the staff member a brief overview also notify the staff members setting manager but with limited details.

Step 2: Investigation Meeting Notification
Investigation meeting invitations should be sent not only to who is under investigation but to all witnesses also. Setting managers will be notified of times and dates to arrange cover.

Step 3: Investigation Meeting Notes
This form is for the note-taker to use during an investigation Meeting. These notes will be used as part of the report that will happen next.

Step 4: Remote Statement
When you need to obtain a statement from someone but cannot meet in person, you can complete this form to obtain their statement remotely.

Step 5: Investigation Report
Once the investigation has been completed, you must complete this report and include all information you think the decision maker will need to decide whether to hold a hearing.

Disciplinary Stage
Step 1: (Optional) No Action Notification
If you are the decision-maker and after looking at the investigation that you feel there is no action at all, this will notify the staff no action will be taken against them.

Step 2: Disciplinary hearing Notification
This is to notify the member of staff that the decision-maker will be carrying out a disciplinary hearing in relation to the investigation that has happened.

Step 3: Disciplinary Outcome Report
Once the hearing has been completed this must be completed within 14 days. This will be where the final decision is to me made in regards to the outcome.

Appeal Stage
Step 1: Appeal Meeting Notification
This form will be carried out by the designated appeal person. This after the decision maker has been made aware an appeal has been requested. This will set time and date for this.

Step 2: Appeal Hearing Outcome
..This is to notify the member of staff what the outcome of of the apeal has been. This must be completed in 14 days. There is not futher action past this point
